Using Youreka’s Form Template Builder, you can drag and drop questions onto the template editor (sometimes referred to as the "canvas"). There are 18 question types that Youreka supports. When a user adds a question to a form template, they are asked to label it, as well as select a question type.
Below is a list of all question types available, as well as description for each.
Attachment/Photo
Attachment/Photo questions give users the ability to attach files and photos to a form.
You can make a photo question required, which forces end users to append a photo/file to this specific question before allowing them to submit forms. If end users are filling out a form on a desktop computer, this question type allows a user to attach a file to the form from the device's local storage. If end users are filling out a form on the Youreka mobile app, this question type allows a user to use the device's camera to capture photos.
When configuring the question, you have the ability to select Auto-Name with Date/Time, which ensures that when the files are attached by the end user, they will automatically be named with the date and time those documents are attached to the form.
You also have the ability to select Allow Einstein Vision Analysis, which enables photos captured for this question to be analyzed by Einstein Vision. Visit Salesforce Trailhead to learn more about Einstein Vision.

Checkbox List
Checkbox lists give users the ability to select multiple answers for a single question. Each option that is added from the template builder is represented as a checkbox when the form is being completed.
Currency
Currency questions can be used to collect information that represents currency. All information collected with this question type is ultimately stored in a Salesforce currency field.
Optionally, admins can set up a value range for questions of this type. Values in the value range can be hardcoded, or relative to other questions in the form.
Date
Date questions can be used to collect information that represents a date. When end users interact with this question in the desktop UI, or on the Youreka Mobile App, they are offered a date picker which allows them to select a date of their choice.

Date/Time
Date/Time questions can be used to collect information that represent date/time. Similar to the Date question type, when end users interact with this question in the desktop UI, or on the Youreka mobile app, they are offered a date picker, as well as a time picker to select a date/time of their choice.
Optionally, admins can set up a value range for questions of this type. Values in the value range can be hardcoded, or relative to other questions in the form.
E-Signature
E-signature question allow users to draw signatures within a form. E-signatures work when filling out forms in the desktop UI, or on the Youreka mobile app. In both cases, users can click on the signature box, and begin drawing.
Multiple signatures can be captured in a single Form by using multiple signature questions.
Responses to e-signature questions are stored as base64 strings in Salesforce.
Email question types allow users to collect email addresses from within a Form. The answers to these questions are recorded in Salesforce as an email data type.
Field Link
The Field Link question type allows users to connect fields on Youreka Forms to fields on objects in the Salesforce environment. For more information on this, visit the Field Link article to learn more.
Geolocation
Use the Geolocation question type when end users are expected to capture their current geolocation and populate an answer with it. This question type can be field linked to custom geolocation fields using the decimal format. The answer will display the Geolocation in Latitude/Longitude separated by a space. Fractional degrees will be shown in decimal format using a period for the decimal (regardless of locale) with exactly 6 digits (so trailing zeroes may be present). This is true even for answers to Field Link Questions where the custom field is specified to use a different format. Standard Geolocation fields (for example Account.ShippingGeolocation) will continue to be unavailable.
Note: Geolocation services for your web browser need to be enabled on your device for the field to populate the data.
Long Text
Use long text fields to collect large amounts of free text on a form. Responses to long text fields are stored in Salesforce as data type Text Area (Long).
Long Text questions are capable of capturing Einstein Sentiment and Einstein Intent information. For more details on these features, visit the Einstein-related articles of this documentation.

Multi-Select Picklist
Use multi-select picklists when users need to select potentially multiple options from a set of picklist values.
Number
Use Number questions for instances when numerical answers are appropriate inputs. Number questions can be used in custom formulas.
Optionally, admins can set up a value range for questions of this type. Values in the value range can be hardcoded, or relative to other questions in the form.
Percent
Use the Percent question type when percent data is the most appropriate input. Percent questions can be used in custom formulas.
Optionally, admins can set up a value range for questions of this type. Values in the value range can be hardcoded, or relative to other questions in the form.
Phone
Use the Phone question type when end users are expected to enter a phone number into the form. Data from this question type is stored in Salesforce as the Phone Number data type.
Picklist
Use the Picklist question type in situations when end users are expected to enter a single value from a structured/limited set of options. Picklists display to end users as a dropdown menu from which they can pick their selection.
Radio Button List
Use the Radio Button List question type when end users are expected to enter a single value from a structured/limited set of options. This question type is similar to picklists, except that it displays to end users as buttons instead of a dropdown menu. While configuring the form template, you can select whether or not the buttons are large or small, as well as which icons are shown inside of each selection option.
Sketch Pad
Use the Sketch Pad question type to allow users to sketch up a portion of the form. They can either sketch on a blank canvas or on top of a template image!
The Sketch Pad question can be dropped into a Standard Section or Linked Section. One added, the user is able to define the title of the Sketch Pad and whether or not it will be shown as a blank canvas or an image in Complete Forms.
Another cool addition is the new Icon Library functionality. The Template Builder has the ability to define icons that can then be used in Complete Forms to add additional context to the Sketch. Think of icons as mini images that can help a non-experienced artist who may be struggling to draw something, like a fire hydrant.
Note: If your user does not have a Template Builder license, the Advanced Data Collection License is required to use the Sketch Pad feature.
Text
Use the text question type to allow users to enter free text into a form. Free text questions also support barcode scanning.
URL
Use the URL question type to allow users to enter URLs into a form. Data from these questions are written back to Salesforce as data type URL.
Yes/No
Use the Yes/No question type to easily populate a question with Yes/No options. By default, when selecting this question type, you utilize the Radio Button List question type, and the picklist options are populated for you.
